By pressing submit, I understand that by submitting this form on or before the 15th day of my last month, I am giving notification to The Music Studio Atlanta to cease lessons and further charges to my account. If submitted past the 15th day of the month, I understand I am still responsible for the next month’s tuition whether or not the student(s) attend lessons. As a friendly reminder, the student may take up to 4 make up classes, or substitute a sibling, parent or friend to satisfy any missed lessons.
